The Volunteer Healthcare Clinic (VHC) is looking to hire a part-time BI-LINGUAL Patient Advocate to provide support with patient registration and patient follow up including case management and patient advocacy efforts. The Patient Advocate provides medical, social service, and community referrals as well as identifies and proactively handles other patient inquiries and needs. The ideal candidate will be a compassionate, detail-oriented person who can work independently – and follow through accordingly – and who functions well as part of a small, close-knit team.
Job Summary / Highlights
- Assists with the patient registration process and collection of donations during evening clinics
- Provides case management services to help patients obtain needed care and/or resources
- Refers patients to programs & services and assists patients with the enrollment process as necessary
- Manages the Chronic Disease Management (CDM) enrollment process and optimizes CDM resources
- Provides information about special VHC programming and coordinates patient sign-up process
- Meets with the parents of pediatric patients to determine eligibility for Medicaid/CHIP program
- Maintains a resource directory and distributes referral information as necessary
- Assists the Patient Services Director and Charge/Case Mgmt. Nurses with follow-up patient care as needed
- Participate in community outreach and resource fairs
- Maintains tracking of all referrals including the type & quantity and assists in reporting efforts
- Reports to the Director of Patient Services
Work Schedule: 20 hours per week / On-site at VHC
Evening Clinics – Monday, Tuesday, Thursday & 3rd Wednesday of the month from 5 pm – 9:30 pm (or until clinic ends)
Case Management – during the daytime on Wednesday and/or Friday or at a time agreed upon with the Patient Services Director
Required Education / Experience / Skills
- MUST BE BI-LINGUAL IN SPANISH (written and spoken)
- Excellent communication, interpersonal, and customer service skills
- Caring individual who can provide culturally appropriate care to people in need of services
- Computer knowledge & skills; ability to learn quickly and become proficient on AthenaNet, VHC’s Electronic Medical Records (EMR) system
- Bachelor’s Degree preferred
- Study in Social Work or related field preferred
- A minimum of 1 year of case management experience / making referrals
- 1 year as a volunteer in agency preferred
