About Our Company
Core Health Metrics IQ is a new and innovative system that provides dedicated care to the Long-Term Care community, specifically to patients residing in their own home. As a new ecosystem we are working to expand our operations nationwide, one state at a time, and currently operate in Pennsylvania and New Jersey localities.
OVERVIEW
Our Care Team plays a crucial role in connecting the patients within the community with our services and other resources. The team consists of two positions: Patient Care Liaison and Patient Care Navigator. These positions require an individual with a passion for public health, community engagement, and some experience/knowledge of the healthcare landscape. The Care Team Liaison serves as an office-based correspondent who outreaches to prospective patients. They also receive inbound calls from patients and Care Navigators.
