    About Our Company

Core Health Metrics IQ is a new and innovative system that provides dedicated care to the Long-Term Care community, specifically to patients residing in their own home. We are an independently owned LTC pharmacy, telehealth physician practice (not open to the general public), and remote patient care facility. As a new ecosystem we are working to expand our operations nationwide, one state at a time, and currently operate in Pennsylvania and New Jersey localities.

    OVERVIEW

Our Care Team plays a crucial role in connecting the patients within the community with our services and other resources. The team consists of two positions: Patient Care Liaison and Patient Care Navigator. These positions require an individual with a passion for public health, community engagement, and some experience/knowledge of the healthcare landscape. The Care Team Liaison serves as an office-based correspondent who outreaches to prospective patients. They also receive inbound calls from patients and Care Navigators.
