    _The anticipated pay range for candidates who will work in California is _$19.00 to $20.00 hourly _. The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc. _

    We are looking for full-time Patient Liaison.

    Responsibilities include:

    • Act as a bridge between residents, families, and facility staff to communicate concerns, requests, and feedback effectively.
    • Conduct regular check-ins with residents and their families to proactively identify and resolve concerns or issues.
    • Advocate for residents by coordinating with interdisciplinary teams to ensure personalized care plans are understood and followed.
    • Conducts the admission process of signing in and explaining admission policies to patients and their families. Ensures a smooth transition is achieved and that all paperwork is complete upon admission. Emphasizes financial arrangements and responsibilities.
    • Document and track resident concerns, complaints, and resolutions to identify trends and recommend process improvements.
    • Develop and implement programs or initiatives to enhance resident satisfaction and engagement.
    • Facilitate resident and family meetings to address concerns, provide updates, and support open communication.
    • Perform other related duties as required.
    • Attends all Center or Company required in-services and meetings.

    Qualifications:

    • Previous experience with geriatric population or in long-term care setting strongly preferred.
    • High School diploma or equivalent required; AA or Bachelor's degree preferred.
    • Ability to represent Company in an ethical and professional manner and to maintain accountability.
    • Must be able to relate to and work with physically and/or mentally ill, disabled, elderly, emotionally upset, and at times hostile people within facility and treat them with respect and consideration regardless of their cognitive or functional level.
