Job Overview:
Monarch Hospice Care is seeking an experienced and compassionate Hospice RN to join our dedicated team in providing end-of-life care to patients throughout Southern California. As an RN , you will play a vital role in overseeing all aspects of patient care, ensuring the highest quality of service while working collaboratively with a multidisciplinary team. Our patients receive care in their homes, assisted living facilities, or nursing homes, so reliable transportation is essential, along with the ability to coordinate visit schedules effectively with both patients and caregivers. Experience with using "HospiceMD" is required.
Responsibilities:
- Provide compassionate care to hospice patients in their homes or healthcare facilities.
- Work with the interdisciplinary team to develop and implement personalized care plans.
- Assess patients regularly, monitor symptoms, and manage pain and comfort.
- Educate patients and their families on disease progression and the hospice care process.
- Coordinate care with physicians, healthcare professionals, and family members.
- Keep accurate and up-to-date patient records.
- Attend regular team meetings and ensure compliance with regulatory standards.
Qualifications:
- Must have an active RN license in California.
- At least 1-2 years of experience in hospice care.
- Solid understanding of end-of-life care, symptom management, and hospice rules.
- Strong communication, assessment, and organizational skills.
- Able to work independently and manage a patient caseload efficiently.
- Compassionate and focused on providing patient-centered care.
