About the Role:

As a Per-Diem Physical Therapist, you’ll play a crucial role in helping patients restore mobility, strength, and independence in their home environment. You will develop personalized therapy plans to support their recovery and improve their quality of life.

Key Responsibilities:

Conduct comprehensive physical therapy assessments and establish realistic rehabilitation goals.

Develop and implement individualized treatment plans, focusing on mobility, strength, balance, and pain management.

Provide hands-on physical therapy services as ordered by physicians, ensuring optimal patient outcomes.

Educate patients and caregivers on therapeutic exercises, fall prevention, and home safety strategies.

Collaborate with nurses, social workers, and other healthcare professionals to ensure comprehensive, patient-centered care.

Maintain accurate and timely documentation following regulatory standards.

Stay current on advancements in physical therapy practices and home health regulations.

Qualifications:

Valid NV State License as a Physical Therapist

Minimum 1 Year of Experience in home health or acute care

Strong clinical skills with a patient-first mindset

CPR Certification (Required)

Excellent communication & interpersonal abilities

Reliable transportation for home visits

Understanding of Medicare home health regulations & documentation
