Per Diem Physical Therapist
Program: Strive Clinic
Location: Holyoke, MA
Salary: $55 per hour (no benefits), with potential to move into a full-time position at $93,600 annually with full benefits
Job Summary:
ServiceNet is seeking a dedicated and skilled Physical Therapist to join our team. The Physical Therapist is responsible for evaluating, re-evaluating, and implementing treatment plans for clients in accordance with established regulations and organizational by-laws. This position involves direct client care, documentation, interdisciplinary communication, and coordination with external providers to ensure the highest quality of rehabilitative services for individuals with acquired brain injuries.
Key Responsibilities:
Client Assessment and Treatment
Evaluate and re-evaluate clients’ functional status per organizational by-laws and regulations.
Develop and implement individualized treatment plans to address client needs.
Treat 8-12 clients per day, providing evidence-based physical therapy interventions.
Monitor and document client progress, ensuring compliance with regulatory standards.
Communication and Coordination
Communicate regularly with Program Directors, nurses, team members, and staff regarding client updates, changes, and recommendations.
Collaborate with rehab coordinators, Occupational Therapists (OTs), and Speech-Language Pathologists (SLPs) to discuss client care and clinic operations.
Coordinate with National Seating and Mobility representatives to schedule wheelchair appointments for clients.
Documentation and Compliance
Complete and maintain accurate documentation in accordance with regulations and organizational policies.
Prepare and submit letters of medical necessity for wheelchair modifications.
Client and Staff Education
Develop home exercise programs tailored to individual client needs.
Distribute exercise programs to designated houses and ensure staff compliance.
Qualifications:
Education: Doctorate or Master’s degree in Physical Therapy from an accredited program.
Licensure: Must hold a current and valid Physical Therapy license in Massachusetts.
Experience: Previous experience in a clinical or rehabilitative setting preferred.
Skills: Strong communication, collaboration, and organizational skills.
Knowledge: Familiarity with rehabilitation equipment and best practices in physical therapy treatment.
Working Conditions:
Ability to work in a clinical setting with clients of varying cognitive and mobility levels.
Must be able to lift/move patients as required for therapy.
Travel between client locations may be required.
