The Allegheny Valley Association of Churches is a non-profit community ministry dedicated to providing quality basic needs services to individuals and families experiencing poverty, food insecurity, and/or homelessness. We are currently seeking an individual to perform 2 part-time positions, creating a full-time position. The first position is 25 hours per week as a Supportive Housing Case Manager to join our Hospitality Homes program. This program is a 20-unit Supportive Housing program which provides services to individuals and families who are experiencing homelessness and have a documented disability that prevents them from maintaining housing without the assistance of a professional. The second position is 15 hours per week and will serve as the receptionist at the front desk ofour ministry center.

Position Summary:
The Supportive Housing Case Manager performs case management-related functions to facilitate and maintain independent housing for individuals and families where at least one member lives with a debilitating disability that may include mental illness, physical condition, substance use disorder, or co-occurring disorders within the Allegheny County Continuum of Care.
The Receptionist will answer phones, provide referrals to callers and direct support to the Office Coordinator.
Job Responsibilities:
Provides outreach, assessment and referral services to residents to ensure access to appropriate housing options and services. Provides necessary housing search/placement, ongoing rental and utility assistance for residents. Responsible for full implementation and direct service delivery to residents through the development of a case plan based on goals identified by the participant. Provides information and support and completes the admission process for persons referred into the Hospitality Homes Scattered-Site Housing program. Responds to and effectively handles emergency situations that may require flexibility of time. Works in cooperation with a multi-disciplinary team and assures continuity of participant services.
Provide receptionist type duties, answering phone calls, making copies, scheduling appointments, and serving as a peer support to the Office Coordinator.
Qualifications:
Candidates will possess at least a high school diploma and three years of experience working in the field of human services/housing services with individuals and/or families diagnosed with psychiatric, medical, and/or substance use. Bachelors or Associates degree and one (1) year related experience preferable. Experience working with those experiencing homelessness and related data bases will be given preference. Must possess a valid Pennsylvania State driver’s license and reliable access to transportation. Must be able to use personal vehicle to provide in-home services as required. Must have or be able to obtyain Act 33 and 34 clearances.
