Position Summary:
Step into a pivotal role as a Transplant Pharmacy Specialist at a leading healthcare facility in Houston. Focused on optimizing care for transplant patients, this position involves tailoring pharmacotherapy plans, collaborating with a multidisciplinary team, and advancing medication safety practices. You’ll also engage in quality improvement projects, mentor pharmacy trainees, and contribute to innovative research in transplant medicine.
Key Responsibilities:
- Oversee complex medication regimens for transplant patients, ensuring safety, effectiveness, and cost-efficiency.
- Partner with healthcare providers to deliver personalized pharmaceutical care, including transitions of care and medication reviews.
- Serve as a mentor and preceptor for pharmacy residents and students, sharing advanced expertise in transplant pharmacotherapy.
- Drive medication-related quality initiatives, including formulary management, medication use evaluations, and safety protocols.
- Participate in research and scholarship to enhance transplant outcomes and contribute to pharmacy advancements.
Qualifications:
- Education: Doctor of Pharmacy (PharmD) from an accredited institution.
- Experience: PGY1 residency completion required, with PGY2 training in solid organ transplant or equivalent practice experience preferred.
- Licensure: Licensed pharmacist in Texas or eligibility for licensure.
- Skills: Strong collaborative and analytical abilities with a passion for transplant patient care.
