Overview

Our client is a well-established skilled nursing facility offering a balanced mix of short-term rehabilitation and long-term care patients. They are seeking a dedicated and skilled Physical Therapist (PT) to join their team and provide top-quality rehabilitation services to help patients regain independence and improve their overall well-being.

Position Summary/Responsibilities

The Physical Therapist (PT) is responsible for assessing, planning, and implementing individualized rehabilitation treatment plans for patients. Key responsibilities include:

• Conducting thorough assessments of patients' physical abilities and impairments.

• Developing personalized treatment plans based on patient assessments.

• Administering therapeutic exercises, manual therapy, and other treatment interventions to improve mobility, strength, and function.

• Educating patients and families on exercises, techniques, and rehabilitation goals.

• Collaborating with the interdisciplinary care team to ensure comprehensive patient care.

• Documenting patient progress and maintaining accurate records in the electronic medical records (EMR) system.

• Providing discharge planning, including recommendations for follow-up care or home exercise programs.

• Ensuring compliance with facility policies and regulatory requirements.

Requirements

• Doctor of Physical Therapy (DPT) degree from an accredited program.

• Current and valid Physical Therapy license.

• Experience in skilled nursing, short-term, and long-term care settings preferred.

• Proficiency in electronic medical records (EMR) systems.

• Excellent communication, patient care, and problem-solving skills.

• Ability to work collaboratively with a multidisciplinary team.

• Strong organizational skills and attention to detail.

• Ability to manage a caseload of patients with various levels of rehabilitation needs.
