Responsibilities
Make a difference for your clients and their families in their own home!
Implementation of all physical therapy modalities to patients
All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization
Improves or minimizes residual physical disabilities of the patient
Returns the individual to optimum and productive level within patient’s capabilities
Participates with all other home care personnel in patient care planning
Reviews initial history and reviews initial evaluation
Consults with physicians regarding patient’s progress
Prepares clinical and progress notes
Helps communicate the plan of care
Consults with family and agency personnel
Ability to do extensive bending, lifting and standing on a regular basis
Qualifications
A graduate of a PTA Program recognized by the American Physical Therapy Association or other National accepted Council or Association
Active license (PTA)
Must pass criminal background check
Must have current CPR certification or willing to complete
Must have active driver's license and clear driving record
Computer skills
Ability to exercise initiative and independent judgment
