Job Description
Provides direct Physical Therapy (PT) treatment under the direction of the Physical Therapist in accordance with the physician’s orders and as outlined in the PT Plan of Care.
Monitors patient’s status and home environment making sure patient is successfully completing treatment as delegated by the registered Physical Therapist.
Reports to registered Physical Therapist any changes in patient’s condition and/or needs.
Maintains communication with other Agency personnel involved in patient’s care to promote coordinated care.
Prepares, submits and maintains accurate, relevant and timely clinical documentation per Agency policy.
Attends mandatory agency staff meetings, care conferences and educational offerings.
Perform administrative tasks such as keeping records, writing reports and making telephone calls.
Maintain current CPR certification, Tuberculosis and all other required licensures.

Assumes the responsibility for personal growth through continuing education and be current with changes in home care, seminars and membership in professional organizations.
The Physical Therapist Assistant would perform skilled direct patient care activities under the clinical supervision of the Physical Therapist.
Qualifications
Graduate of an approved Physical Therapy Assistant program
Current or eligible for state license
Prefer active membership with the American Physical Therapy Association (not required)
Prefer minimum one (1) year experience
