Responsibilities:
Conduct comprehensive assessments of physical capabilities, medical histories, and functional limitations to establish baseline data and formulate individualized treatment plans.

Educate their caregivers on proper techniques for performing therapeutic exercises, using assistive devices, and implementing strategies to prevent injuries and promote optimal physical function.

Qualifications:
Graduate of an accredited physical therapy program with a Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) degree.
Valid state licensure as a Physical Therapist, with current CPR certification.
Prior experience in home health, geriatrics, or rehabilitative care preferred.
Strong clinical assessment and critical thinking skills, with the ability to develop and implement individualized treatment plans based on thorough evaluations.
Excellent interpersonal and communication skills, with the ability to establish rapport with patients, caregivers, and interdisciplinary team members.
