    Key Responsibilities:

    • Collaborate with a multidisciplinary team to develop and deliver individualized care plans.
    • Perform assessments, provide therapy, and document patient progress.
    • Educate patients and caregivers on therapy techniques and assistive devices.
    • Travel locally to patients' homes to provide care.

    What We're Looking For:

    • A Bachelor's or Master's degree in Physical Therapy from an accredited program.
    • Current state licensure as a Physical Therapist.
    • At least one year of healthcare experience.
    • A valid driver's license with the ability to travel locally (approximately 20% of your day).
    • Strong organizational skills to manage schedules and meet charting deadlines.
    • Familiarity with mobile devices, tablets, and EMR software.

    Schedule and Shift Details:

    • Monday through Friday, 8:00 AM – 5:00 PM.
    • One weekend per month and two holidays annually.

    Travel:

    • Local travel to patients' homes is required.
