Key Responsibilities:
- Collaborate with a multidisciplinary team to develop and deliver individualized care plans.
- Perform assessments, provide therapy, and document patient progress.
- Educate patients and caregivers on therapy techniques and assistive devices.
- Travel locally to patients' homes to provide care.
What We're Looking For:
- A Bachelor's or Master's degree in Physical Therapy from an accredited program.
- Current state licensure as a Physical Therapist.
- At least one year of healthcare experience.
- A valid driver's license with the ability to travel locally (approximately 20% of your day).
- Strong organizational skills to manage schedules and meet charting deadlines.
- Familiarity with mobile devices, tablets, and EMR software.
Schedule and Shift Details:
- Monday through Friday, 8:00 AM – 5:00 PM.
- One weekend per month and two holidays annually.
Travel:
- Local travel to patients' homes is required.
