Salary: Up to $90,000 per year (based on experience)

Job Overview:

We are seeking a compassionate and skilled Physical Therapist to join our Home Health team in Gwinnett County, GA. Key Responsibilities:

• Patient Care: Provide direct physical therapy services to patients in their homes, including assessment, treatment planning, and intervention based on individual needs.
• Evaluation and Assessment: Conduct thorough evaluations of patient mobility, strength, coordination, and function. Develop personalized treatment plans to help patients regain optimal physical function.
• Treatment Implementation: Implement physical therapy interventions, including exercises, manual therapy, and other techniques, to improve mobility and reduce pain.
• Collaboration: Work closely with the interdisciplinary care team (nurses, social workers, and physicians) to ensure comprehensive patient care and positive outcomes.
• Documentation: Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with company policies and regulatory requirements.
• Patient Education: Educate patients and caregivers on exercises, safety techniques, and other home care instructions to improve patient outcomes.
• Compliance: Adhere to all applicable federal, state, and local regulations, as well as company policies and standards of care.

Qualifications:

• Education: Graduate of an accredited Physical Therapy program.
• Licensing: Clear and active Physical Therapist license in Georgia is required.
• Experience: Previous home health experience is preferred but not required.
• Skills: Strong clinical knowledge, excellent communication skills, and the ability to work independently.
• Transportation: Reliable transportation and a valid driver’s license are required for travel to patient homes within the designated service area (Gwinnett County).
• Other: Must be compassionate, patient, and dedicated to improving the lives of patients through therapy.
