Our client is hiring a Physical Therapist to join their dynamic Home Health team. In this position, you'll play a vital role in delivering high-quality therapy services directly to patients in the comfort of their homes. Your expertise and care will empower patients to achieve their goals and live more fulfilling lives, all while working in a supportive and flexible environment.

Key Duties and Responsibilities

• Conduct patient evaluations and develop personalized treatment plans.
• Document therapy sessions and patient progress, updating the care plan as needed.
• Educate patients and caregivers on therapy exercises and equipment usage.
• Provide supervision to physical therapy assistants and, when applicable, certified nursing assistants.
• Prepare detailed reports and actively participate in team development initiatives.

Qualifications

Required:

• Master's degree or higher in Physical Therapy.
• Minimum of 1 year of experience in a healthcare setting.
• Active Physical Therapist license in Illinois (IL).
• Valid driver's license.

Work Schedule

• Monday to Friday, 8:00 AM–5:00 PM or 9:00 AM–5:00 PM.
• One weekend per month (day shift with scheduled patients).
• Two holidays annually (one summer and one winter holiday).
• No on-call responsibilities.

Travel

• Frequent local travel to patients' residences is required.
