Qualifications:

Master's degree or higher

Valid Physical Therapist license in Illinois (IL)

Valid Regular Driver License (DL)

Minimum of 1 year of experience in a healthcare setting

Preferred:

Physical Therapist license number

Role Overview: Join our Home Health team as a Physical Therapist, where you will be responsible for delivering exceptional Physical Therapy services to patients in their homes. Your role involves assessing patient functioning levels, designing tailored treatment programs to enhance independence and physical/cognitive functioning, and providing education and supervision to patients and caregivers.

Key Responsibilities:

Conduct Physical Therapy assessments and develop individualized treatment programs

Document and communicate patient's response to treatment and any changes in condition

Educate patients and caregivers on therapy programs and equipment usage

Supervise physical therapy assistants and, as necessary, certified nursing assistants

Prepare reports and contribute to staff and Agency development activities

Schedule and Shift Details:

Monday to Friday; 8:00 am/9:00 am - 5:00 pm

1 weekend per month (Saturday & Sunday; day shift with scheduled patients)

2 holidays (1 summer / 1 winter); no on-call required

Travel:

Local travel to residential homes
