    Key Responsibilities:

    • Provide direct rehab care in physical therapy upon physician referral, following all applicable laws, regulations, and company standards.
    • Conduct comprehensive patient evaluations to determine therapy needs.
    • Develop and implement individualized treatment plans in collaboration with interdisciplinary teams.
    • Administer therapeutic exercises, manual therapy, and other modalities.
    • Educate patients and families on treatment protocols and at-home exercises.
    • Oversee and evaluate care provided by PTAs, Rehab Aides, and PT and PTA students.
    • Maintain accurate and timely documentation using therapy software.
    • Ensure compliance with federal, state, and company standards and regulations.
    • Participate in community health projects as appropriate.

    Qualifications:

    • Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT).
    • Current and active Physical Therapist license in Massachusetts, maintaining it in good standing.
    • Preferably one year of experience in post-acute care or a related setting.
    • CPR certification upon hire or during orientation, maintained throughout employment.
    • Proficiency in Microsoft Word, Excel, and email.
    • Strong interpersonal, communication, and independent decision-making skills.
    • Knowledgeable of post-acute care regulations and guidelines.
    • Commitment to patient-centered care and professional integrity.

    Essential Functions:

    • Follow and adapt physical therapy treatment plans for patients.
    • Establish, assess, and modify realistic, measurable, timely, and functional goals.
    • Provide excellent customer service with a positive attitude.
    • Assist in patient evacuations if necessary.
    • Demonstrate dependable attendance and effective reasoning and judgment skills.
    • Communicate and function effectively within an interdisciplinary team.
    • Perform physical tasks such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, and reaching during working hours.
    • Proficiently read, write, speak, and understand English.
