    A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager. This is a PRN position.

    What you’ll be doing:

    Provide physician prescribed physical therapy.

    Improve or minimize residual physical disabilities of the patient.

    Return the individual to optimum and productive level within the patient’s capability.

    Participate with all other home care personnel in patient care planning.

    Direct and supervise personnel as required.

    Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.

    Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.

    Currently licensed in Indiana

    Two (2) years’ experience preferred in a home health setting

    Medicare OASIS documentation preferred

    Valid state driver's license and reliable automobile, current automobile insurance, and willingness to operate personal automobile as needed, based on the nature of the job.

    Successfully pass drug screening, background/medical checks, and motor vehicle record screening.
