Compensation: $36.00-$45.47 (DOE).
Location: Central Oregon
Schedule: Part-time.
Travel: Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided.
What You Get to Do:
Conduct thorough initial assessments of patients and their families to determine home health needs.
Create, monitor, and adjust individualized care plans with measurable goals based on therapy assessments, patient abilities, and diagnoses.
Administer appropriate preventative and rehabilitation treatments.
Educate patients, families, and caregivers on physical therapy instructions and home exercise programs (HEP).
Actively participate in the Fall Intervention Program.
Address discharge-planning needs in care plans.
Update the Home Health Aide Plan of care as needed.
Supervise and evaluate home health aide care every 14 days.
Attend staff meetings, case conferences, and IDT sessions as required.
Required Skills & Qualifications:
A degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
Licensed to practice as a physical therapist within the state.
Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
Ability to pass a background test, drug screen, and TB test.
Valid CPR Certification.
Current automobile insurance, valid Oregon driver’s license, and reliable transportation.
Strong communication and interpersonal skills.
Ability to work independently and collaboratively within an interdisciplinary team.
