Job Summary
The Physical Therapist is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients as prescribed by the attending physician.
Responsibilities
Comprehensive evaluation of the patient's level of function and participation in the development of the total patient care plan in conjunction with RN (if applicable), patient, family, and physician.
Participation in case conferences/ team huddles. Ensure proper documentation.
As needed by the agency, supervision of Home Health Aides and when such personnel is participating in the patient's therapy regime.
Instruction of patient, family, and other agency health care personnel in the patient's treatment regime when indicated
Assists patients to reach a level of function to enable them to remain safely in their home environment while reaching national standards for outcomes and maintaining fiscal responsibility
Looking for someone to perform in a PART-TIME or PER-DIEM capacity
Qualifications
Graduate with at least a baccalaureate degree from an accredited school of Physical Therapy approved by the American Medical Association Council of Medical Education in collaboration with the American Physical Therapy Association.
Current license to practice as a Physical Therapist
At least two (2) years of clinical experience.
Knowledge of physical therapy methods with particular skills in casework in the home setting.
