    Physical Therapist (PT) / Physical Therapy Assistant (PTA) – Home Health (Per Diem)
    Location: Boca Raton, FL
    Employment Type: Per Diem

    We are seeking a Per Diem Physical Therapist (PT) or Physical Therapy Assistant (PTA) to join our Home Health team in Boca Raton, FL. This role offers a flexible schedule and the opportunity to provide high-quality rehabilitative care in a home setting.

    Responsibilities:

    • Conduct in-home patient evaluations and implement individualized treatment plans (PTs).
    • Assist in executing prescribed therapy programs under the supervision of a licensed PT (PTAs).
    • Provide therapeutic exercises and interventions to improve mobility, strength, and overall function.
    • Educate patients and caregivers on home exercise programs and safety measures.
    • Collaborate with an interdisciplinary team to ensure optimal patient outcomes.
    • Accurately document patient progress and treatment plans in an EMR system.
    • Maintain compliance with state, federal, and agency guidelines.

    Requirements:

    • Active Florida PT or PTA license.
    • Home Health experience preferred.
    • Familiarity with OASIS documentation and EMR systems (such as HomeCare HomeBase or Kinnser WellSky) is a plus.
    • Strong interpersonal and communication skills.
    • Ability to work independently and manage a flexible schedule.
    • Reliable transportation for patient visits.
