Physical Therapist (Home Health)

We’re looking for a compassionate and driven Physical Therapist to join our growing home health team. If you’re passionate about helping patients regain mobility and independence in the comfort of their own homes, this is the role for you.

What You’ll Do:

• Evaluate and assess patients' physical conditions

• Develop and implement personalized treatment plans

• Educate patients and families on exercises and home care techniques

• Monitor progress and adjust therapies as needed

• Document treatment plans, interventions, and outcomes

• Assist patients with mobility aids (e.g., walkers, wheelchairs)

What We’re Looking For:

• Graduate of an accredited Physical Therapy program

• Valid state license to practice

• Minimum 1 year of PT experience (Home Health experience preferred)

• Strong communication skills and a caring, patient-first mindset
