    The Provider Relations Outreach Associate is responsible for marketing and education in the community providing coordination of community resources for clients served by Professional Nurse Partners and Integrative Health Management with a clear customer focus. The Provider Relations Outreach Associate achieves customer satisfaction while developing and implementing a strategic marketing plan.

    Our life care management and wellness program sales representatives present the benefits and appropriate use of home care to physicians, hospital case managers, skilled nursing facility social services and other healthcare professionals. To enable our representatives to effectively present life care management and home care, we provide formalized training and support while empowering our representatives to exercise the entrepreneurial responsibility they need to succeed. We are dedicated to the success of our team professionals and provide tools and support to help them achieve their goals. Responsibilities include:

    · Conduct 12-15 sales call per day to area physicians and other healthcare professionals

    · Conduct educational in-services to promote our life care management and in home care support services

    · Create and maintain a consistent and efficient territory planner

    · Update account information in CRM (customer relationship management) software

    · Communicate daily with the clinical operations staff to foster an environment of teamwork

    · Complete required weekly reports in a timely manner.

    · Maintains complete & accurate referral sources profiles.

    · Follow Professional Nurse Partner’s marketing procedures as outlined in the marketing orientation manual.

    · Acts as a liaison between clients and organization’s staff to resolve problems, provide up to date information, and maintain positive customer relations.

    · Visits with physicians, discharge planners, estate planners, elder law professionals and other related health-care providers regarding service provisions, contracts and communication with organization.

    · Makes marketing presentations to current and prospective clients to educate them on innovative programs & services.

    · Maintains a networking base in the community through trade associations, business organizations and local association meetings.

    · Is flexible and persistent and takes initiative to effect positive changes.

    · Collaboratively communicates/consults with the licensed professionals, internal team and community partners

    · Attends meetings, health fairs and community events as requested by the Administrator

    · Promotes Professional Nurse Partner’s mission and vision within the community.

    · Projects concerned, professional attitude and appearance toward organization’s staff, referral sources and general public.

    · Maintains confidentiality of patient/client and agency matters.

    · Submits documentation timely.

    · Performs other duties as assigned by his/her direct supervisor.

    Requirements:

    College degree and/or equivalent sales experience required

    2-3 years sales experience in home care, hospice, or other medical sales is a plus

    Excellent written, communication and presentation skills necessary

    Must have a proven, successful sales track record

    Current California Driver's License Required

    Must be authorized to work in the United States
