As a Service Facilitator you play an essential role in coordinating mental health services for children in Kenosha County. Our goal is to keep families in tact by developing supportive teams and increasing independent functioning. Service Facilitators are responsible for conducting initial mental health assessments, designing treatment plans, making referrals, safety planning, leading team meetings, providing emotional support and responding to crises.
Service Facilitators can also expect to be a part of program-wide activities, groups, outings and special events for our consumers, and to join a team of supportive and caring coworkers. KEY RESPONSIBILITIES:
Interview and evaluate clients and families to complete intake and assessment process (CANS and Functional Screen).
Complete intense, Medical Assistance (MA) compliant Strengths and Needs assessment of child and family.
Assess client situations, capabilities and needs to determine services required to meet treatment goals of the client and their family.
Develop, implement, evaluate and revise outcome oriented and strength based treatment plan (ISP) as changes occur.
Develop a family support team comprised of formal and informal supports.
Facilitate team meetings with service providers, family members, and family supports in which progress of goals and services are monitored.
Develop a crisis/safety plan for the client and family.
Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
Attend client/family court proceedings, IEP meetings, psychiatry appointments, or other appointments which may require family support or advocacy.
Incorporate knowledge and philosophy of Coordinated Services Team (CST) into daily work with clients. Become certified to conduct initial and annual screens to determine eligibility for Comprehensive Community Services, Children’s Long Term Support Waiver, and Children’s Community Options Program funding benefits for all program clients.
Utilize all the available MA funding benefits such as Targeted Case Management, Children's Long Term Support Waiver, Comprehensive Community Services, and Children’s Community Options Program while complying with their specific regulations.
Utilize advanced knowledge of services and resources in the community and be willing to research new and unknown resources to assist the client and family in meeting their goals.
Work in collaboration with other providers, school personnel, mental health professionals, Division of Children and Family Services workers, and court officials.
Advocate for and teach advocacy skills to client and family while attending team meetings, school/IEP meetings, Division of Children and Family Services staffings, and court hearings.
Formulate daily contacts into units entered into various databases.
Meet all deadlines of documentation including entering progress notes, reviewing provider progress notes, assessments, individual service plans, safety plans, service authorizations, quarterly reports, annual re-certifications, and discharge summaries; this includes allowing time for Supervisor and Clinical Services Director to review these documents.
Administer surveys to families to collect data on family satisfaction.
Meet requirements of state reporting and outcomes reporting through the use of Program Participation Survey (PPS) and the Families First Sharepoint Survey.
Participate in weekly program staffing and present assigned cases.
Receive clinical supervision and implement recommendations into treatment plan.
Report client behavior to courts and Kenosha County Department of Human Services and make appropriate recommendations.
Attend trainings to keep abreast of new information related to the mental health field.
Ability to operate a motor vehicle safely and obey all traffic laws. Must have reliable transportation on a daily basis, a valid driver’s license and automobile insurance.
Safely transport clients as needed.
