    Job Title : Program Coordinator

    Job Summary Renewed Vision CDDC is seeking a dynamic and detail-oriented Program Coordinator to lead and oversee both our Residential Program and Shared Living Providers. This position is essential to our mission of empowering individuals with developmental disabilities to lead meaningful, independent lives. The Program Coordinator will ensure high-quality care, compliance with regulations, and successful collaboration between staff, service providers, and families.

    Key Responsibilities

    1. Program Oversight * Supervise and manage both residential programs and Shared Living Providers to ensure safe, supportive, and inclusive environments for all individuals served. * Develop, implement, and monitor individualized care plans tailored to the needs and goals of service recipients in all programs. * Ensure compliance with state, federal, and organizational policies and regulations across both programs.
    2. Shared Living Program Management * Hire, train, and evaluate Shared Living Providers to ensure alignment with organizational values and quality standards. * Act as the primary point of contact for Shared Living Providers, addressing concerns, providing guidance, and ensuring they have the resources to succeed. * Conduct regular home visits and evaluations to ensure quality of care and compliance.
    3. Staff Management * Hire, train, and supervise residential program staff, creating schedules and ensuring proper coverage. * Provide coaching, guidance, and performance feedback to staff and Shared Living Providers. * Foster a culture of respect, professionalism, and teamwork across all program areas.
    4. Client Support * Build and maintain positive relationships with individuals served, their families, and support networks. * Address and resolve crises or concerns using a person-centered approach. * Advocate for the needs and goals of individuals within the organization and the broader community.
    5. Administrative Duties * Maintain accurate and timely documentation, including case notes, incident reports, home evaluations, and program metrics. * Develop and manage program budgets, ensuring resources are allocated effectively. * Participate in organizational meetings, training sessions, and quality assurance initiatives.

    Qualifications

    • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (equivalent experience may be considered).
    • Minimum of 2 years of experience working with individuals with developmental disabilities, including program management or supervision.
    • Knowledge of Shared Living models and person-centered planning principles is highly desirable.
    • Strong leadership, communication, and organizational skills.
    • Proficiency in documentation systems and standard office software.

    Physical Requirements

    • Ability to perform moderate physical activity, including lifting up to 50 lbs.
    • Must possess a valid driver’s license and reliable transportation for home visits and work-related travel.

    Work Schedule This is a full-time position with occasional evening, weekend, or on-call responsibilities based on program needs.
