Able Health Services Inc. is seeking a Coordinator of Community Services Program Director to join our team!
This position is responsible for: The Program Director is a critical position that ensures services provided by each Coordinator of Community Service is of the highest quality and that Coordinators of Community Service are well supported to carry out their job responsibilities.
Job Description:
• Oversee day-to-day operations
• Intervene when services are needed in times of crisis and/or outside the regular work schedule.
• Execute procedures as outlined in the Able Health Services Inc. Policy and Procedure Manual,
• Manages Coordinators of Coordinator of Community Services (CCS) performance to ensure work meets quality person-centered Standards.
• Ensures that Coordinators of Community Services (CCS) are using person-centered practices and principles in all aspects of their role including language and actions with the people supported and external and internal Team Members
• Coordinate/implement individual satisfaction survey
• Serve as an advocate for each individual’s well-being, human and legal rights, and in the individual’s best interest at all times.
• Participate in regularly scheduled meetings and training as required for execution of job duties and as requested by the DDA, and Able Health Services Inc.
• Perform additional coordination of community service duties as assigned by the Able Health Services administrative staff and the DDA, such as resource allocation for the resource library.
• Monitors Key Performance Indicators (KPIs)
• Completes required Comprehensive Quality Review (CQR) and monthly Quality Assurance Data for oversight and performance evaluations. Completes one full review for each Coordinator of Community Services (CCS) supervised per month (CQR)
• Ensures Performance Utilization (PU) monthly benchmarks are met.
• Conducts hiring interviews for vacant CCS positions and uses the rating scale and interview scoring procedure to inform decisions.
• Conducts 6-month and annual performance evaluations for new Team Members
• Conducts annual performance evaluations for Coordinator of Community Services (CCS) Team Members over 1 year.
• Utilizes situational leadership to develop Team Members knowledge and skills.
• Takes disciplinary actions consistent with AHS policies and procedures to address performance concerns.
• Recommends to appropriate CCS Program Director, termination of Coordinator of Community Services (CCS) when performance does not meet job expectations.
• May provide support for a small number of people supported; providing necessary functions described in the Coordinator of Coordinator of Community Services (CCS) job description.
• Coordinates and assigns new Development Disabilities Administration (DDA) referrals, out of office transfers and reassignments to maximize the Coordinator of Community Services (CCS Team Member's workload effectiveness
• Updates, tracks, and monitors assignments in the Coordinator of Community Services (CCS database and LTSS
• Records and maintains supervision records/tracking performance evaluations for developmental feedback and performance evaluations.
• Reviews the Quality Assurance Data for compliance standards every month and follows supervisor procedures.
• Participates in organizational and program leadership meetings to stay informed of changes affecting policies, procedures, and practices.
• Actively participates in work groups and committees by AHS procedures.
• Conducts and/or participates in ongoing meetings: Supervisor supervision, Program Manager Team meetings, supervisory team meetings, and supervision meetings to stay informed of changes affecting processes, procedures, and practices.
• Communicates AHS philosophy to agencies, the people we support and their families and service providers.
• Represents AHS in service providing negotiations, task forces, and community education activities.
• Speaks to community groups to explain and communicate AHS purposes, programs, and policies.
• Forms cooperative relationships with external stakeholders
• Participates in and organizes external task forces and work groups.
• Supports Coordinator of Community Services (CCS) training by reinforcing topics covered in New Team Member Training, reviewing AHS policies and procedures, coordinating shadowing opportunities, observing, identifying, and delivering training based on strengths and areas in need of development.
• Performs all duties as described and others as required, using the foundation of AHS philosophy and values.
