Fresh Start Therapeutic Services (FSTS) is seeking full-time Psychiatric Rehabilitation Program (PRP) Counselors. The PRP Counselor will be responsible for providing on and off-site (in-home/community) psychiatric rehabilitation services to youth and adults with chronic mental illness. The PRP Counselor focuses on services that promote independence in activities of daily living, community living skills, and maintenance of health. This position requires driving. A valid driver’s license, motor vehicle clearance, and proof of auto insurance are all required at time of employment and must be maintained throughout employment. Must be able to work a flexible schedule, including day, evening, and some weekend hours.
Duties:
Provide on and off-site psychiatric rehabilitation services addressing individual client needs targeted to specific diagnostically related behaviors which interfere with independent activities of daily living, community living skills and client ability to maintain health
Participate in regularly scheduled individual and group supervision
Support Individual Care Plan goals and objectives developed by the client’s licensed provider and the development of individual rehabilitation goals and objectives for each client.
Maintain client progress notes and Treatment Plan in Electronic Health Record, indicating progress toward goals.
Maintain informational notes related to client’s PRP services.
Attend and participate in required in-service and other training programs.
Assist with linking clients to all community services for which they may be eligible.
Provide crisis intervention and conflict de-escalation as needed.
Utilize appropriate interventions to assist clients with reaching client’s identified goals.
Qualifications:
Must have a high school diploma. Associates degree or higher is preferred.
Must be able to successfully pass a drug test and background check.
Familiarity with and/or sensitivity to chronic mental illness, co-occurring disorders, and clients from culturally diverse backgrounds (including religious, ethical, cultural and social values).
At least two years of experience providing PRP or Case Management services preferred but will provide training and educational opportunities to enhance clinical skills.
