Job Summary
Holy Angels Residential Facility is in search of a QIDP/Qualified Intellectual Disabilities Professional-Case Manager to provide all case management services and serve as an advocate for all residents on the caseload, and to see that all the residents needs are met. The qualified candidate will have a passion for helping individuals with intellectual and developmental disabilities. The QIDP- Case Manager role includes the following:
Specific Skills and Responsibilities
Maintain close communication with Habilitation Director and other professionals, as well as Supervisors of all shifts, DSPs, and other ancillary services providers to ensure continuity of services
Oversee the day to day needs of the residents
Observe policy of confidentiality in all cases.
Report instances of real or suspected abuse or neglect to the Administrator and Habilitation Director immediately
In-service residents quarterly on their rights and responsibilities
Coordinate with Dorm Supervisors to ensure that residents are receiving continuous active treatment in the manner specified by the Individual Program Plan
Provide in-service training as required regarding documentation or other related matters
Provides assistance to managers when requested by Habilitation Director This may include occasional weekends and normal hours (8:00-4:30) work
Accurately completes all required paperwork within time limits set by the Habilitation Director. This includes, but is not limited to: IPPs, monthly reports, quarterly reports, active treatment schedules, and monthly reports for the Habilitation Director
Chair meetings for all annual staffing on your caseload
Attend all scheduled meetings as requested by the Habilitation Director or other professional staff.
Assist Habilitation Director in planning and carrying out social events, leisure/recreation activities, program planning and other departmental activities
Assist Habilitation Director in matters relating to annual survey process. See that all Habilitation aspects of the survey process for your area(s) are complete and in proper order for the survey
Coordinate with families, staff, and other team member to ensure program consistency and efficacy
Performs all other duties as requested by the Director of Habilitation
Qualifications:
Must maintain a current and valid driver’s license
1-3 Years working with individuals with intellectual and developmental disabilities. 1 year min. exp. REQUIRED
Education:
Bachelor’s Degree in Human Services/ Social Sciences (Psychology, Sociology, Social Work etc. ) REQUIRED

Physical Demands:
Walk, stand, sit, push, pull, stoop, kneel, crouch, reach, use hands and lift up to 50lbs, 2/3’s of the time
