    • Full Job Description:*

    Provide assistance to clients with daily living tasks such as behavior management, counseling services to the client, budget management, independent transportation assistance, time management skills and other tasks to promote independent living

    Provides case management functions of linking, arranging, and advocating for services and referrals to clients.

    Provides support for crisis services for adults and adolescents through face-to-face engagement.

    Maintains all client documentation in the electronic record and writes progress notes that are adhering to the requirements set by the Virginia Department of Behavioral Health and Developmental Services (DBHDS).

    Utilize effective listening skills to establish rapport with the client, family members, and other individuals close to the client; provide emotional support and encouragement.

    Maintains up-to-date client records in accordance with regulatory/agency standards and prepares required necessary reports in a timely manner.

    Educate client and family members regarding existing community services and acts as an advocate in connecting them to these services.

    Be on call to respond to crisis on a 24 hour, seven days a week basis.

    Must have at a minimum Bachelors Degree in Sociology, Psychology, Social Work, or Human Services.
