    Tasks and Responsibilities: Clinical Services are primarily to be provided via face-to-face involvement with the individual and other agencies for appropriate care coordination. The QMHP is required to assist in developing and implementing the person-centered, recovery-based treatment plan as a step-down service from hospitalization and transitioning into the community. The QMHP provides services for adults struggling with acute crises of a psychiatric nature that may jeopardize their current community living situation or puts them at risk of psychiatric hospitalization. This service’s objectives are to prevent exacerbation of a mental health condition, to prevent injury to the individual or others, and to provide treatment in the context of the least restrictive setting. The activities associated with the primary responsibility of this QMHP A/C are:

    Averting psychiatric hospitalization or re-hospitalization by providing guided support for up to 8 hours in one day.

    Assist in the development and ongoing monitoring and implementation of the Individual Service Plan.

    Provide quality living environment with a high assurance of safety and security by working with the identified plan at the level of care needed and supports on safety planning.

    Stabilizing individuals in psychiatric crisis through short-term focused goal.

    Mobilizing resources from the community, support systems, family members, and others for ongoing maintenance, and rehabilitation.

    Maintain continuous contact with the identified individual and other key stakeholders (with signed authorization to release information) in planning and service delivery. These stakeholders might include but are not limited to social workers, psychiatrists, court officials and police, teachers, and other human service professionals.

    Educate individual and family members regarding existing community services, and act as an advocate in connecting them to these services.

    Be on call to respond to crisis on 24 hours, seven day a week basis.

    Maintain up-to-date individual records and prepare required/necessary reports in a timely and accurate manner.

    The QMHP is required to provide service coordination activities for the ISP development and implementation as such they must have the following knowledge skills and abilities:

    Primary models and techniques for treatment of individuals for the purposes of crisis prevention and intervention.

    Working knowledge of community systems, resources, and interagency networking.

    Ability to assist individuals in developing daily and community living skills.

    Basic knowledge of voluntary and involuntary admission to state facilities such as local jails or

    correctional facilities.

    Knowledge skills and ability for symptom monitoring and management.

    Ability to assist individuals in developing behavior and anger management skills.

    Ability and skills to maintain required confidentiality regarding individual case materials in compliance with the Commonwealth of Virginia Human Rights Code 2007 and Health Insurance

    Portability and Accountability Act (HIPAA)

    Ability and skills to work with adults who have serious emotional disturbances and who may be

    hostile, oppositional, and volatile, while at all times maintaining composure and behave in a dignified manner.

    Knowledge about mental health treatments for serious mental illnesses.

    Ability to assess needs and develop appropriate intervention or initial treatment strategies.

    Ability to accurately prepare and maintain individual records and write reports and summaries.

    Academic/Work Experience: Bachelor’s degree in relevant human services field plus at least one year of experience to meet Qualified Mental Health Professional according to DMAS requirements preferred which includes professional, clinical experience in the provision of crisis case management and prevention, or a combination of education, training, and experience indicating the possession of essential knowledge, skills, and abilities; and must be able to transport/drive provide own transportation as necessary.

    QMHPs are required to use their personal vehicles for visits and for engaging in community activities. Therefore, at the time of hire QMHPs must provide proof of driver’s license and insurance. Should a QMHP receive a moving traffic violation at any time after employment the QMHP must notify his or her immediate supervisor about the incident the next business day after receiving the violation. Further, the QMHP will be taken off the schedule in the event they have any lapse in automobile insurance coverage.
