Qualified Developmental Disabilities Professional
Some travel is involved. Meetings can occur in Richmond, Virginia Beach, Suffolk, Norfolk, Newport News, Hampton, Chesapeake, etc.
General Responsibilities: The individual will be responsible for providing QDDP Services for adults and children with intellectual and developmental disabilities.
The QDDP assumes responsibility for the following roles to include:
· Records management
· Records review/audits
· Conducting and approving assessments
· Prescreening, admitting, and discharge planning
· Intake screening and associated paperwork
· Attend Intake, SIS, and Annual meetings
· Overseeing the development and implementation of Individualized Service Plans (Part V Initial and Annual)
· Writing reports such as Quarterly Service Reviews
· Reviewing documentation on the individuals
· Coordinating services with Support coordinator to include scheduling meetings
Knowledge, Skills, and Abilities:
· Experience managing services for adults and children with intellectual, developmental, physical, and mental disabilities
· Experienced in personnel management, HIPPA and records management, developing and implementing Individualized Service Plans, conducting, and approving assessments, prescreening, and discharge planning
· Familiarity with regulations governed by DBHDS and Offices of Human Rights, Medicaid, Department of Labor, OSHA, Community Service Boards, Case Management Services, First Aid, and CPR
· Effective written and oral communication skills
· Strong documentation skills
· Knowledge of behavioral plans
Qualifications:
· A Bachelor’s degree plus one year of experience and supervisory skills working with the intellectually and developmentally disabled population served
· Must have a valid Virginia driver’s license
· Must have one (1) year of working with the intellectually and developmentally disabled population
· Certifications are preferred for Therapeutic Options, First Aid, and CPR. Will provide this training if necessary.
