DESCRIPTION:
This position involves the provision of an array of psychoeducational services in the agency’s Virtual Residential and Home and Community Based Programs as recommended by Children’s Services Act (CSA) Family Assessments Planning Team (FAPT). a within the home and community settings as determined by the client’s treatment plan and under the direct supervision of the Clinical Staff Supervisor.
Virtual Residential Program serves as a diversion for residential treatment that provides intensive client and family focused services to children and adolescents ages 4-20 in the comfort of their homes. This approach to home-based services is designed to create positive change in the client’s behavior, improve symptoms, provide family, and parent support, and strengthen family systems by providing the structure of a formal residential setting within the family’s home and community. The focus of VRP is to reduce out of home placements, increase family engagement, and facilitate successful outcomes.
Home and Community Based Counseling services combine evidence-based therapeutic interventions, direct support in both the home and community setting to address mental health and behavioral challenges as well as situations that may arise with youth resulting in out of home placements. QMHPs are trained in a variety of methods to best support clients and their families experiencing the above noted obstacles. EZETT fosters collaboration and a wraparound approach to assist families with bringing about stability and positive change. Bilingual applicants are strongly encouraged.
TASKS:
Provide weekly hours as recommended by FAPT and as identified in individualized assessment.
Participate in treatment and discharge planning, provide ongoing monthly reports following the approved format, and monitor progress as needed to revise treatment plan to reflect client progress and confirmed needs.
Have monthly contact with the Referring Agency.
Arrange for weekly case supervision with clinical supervisor.
Act as a co-leader of parenting and peer education/support groups as needed.
Act as the liaison between family, referring agencies, and other community organizations.
Coordinate client services between various agencies.
Maintain case records and document services rendered.
Attend weekly staff meetings consistently. Prior approval from management team is required when absent.
Prior to the end of each quarter service period, coordinate and facilitate a staffing for the client, inviting the family of the client, the Referral Source, representation from other pertinent agencies providing services and supervision.
Maintain high degree of client confidentiality.
Work with other staff assigned to cases, communicating effectively.
Maintain a high degree of professionalism in the community, with clients, and with referring agencies seeking to build and maintain positive relationships.
Provide Individual and Family psychoeducational counseling services to clients in the home, school and community setting.
Provide Crisis Intervention as needed.

MINIMUM QUALIFICATIONS:
Candidate must have a bachelor’s degree in an approved Human Services and or Related Field. Experience working with children, adolescents, and or families preferred. Must be registered as a QMHP-A/C/T with the Department of Health Professions Board of Counseling. Must have a mobile phone, Driver’s License, and reliable transportation.
Service Location: Richmond, Hanover, Henrico, Chesterfield, Petersburg, Prince George, Hopewell, Powhatan and Goochland.
