    Job Description: MHSS and Crisis Stabilization Service

    Report to: Chief Executive Officer Supervision: Clinical Director

    Job Summary: Responsible for providing face-to-face services to clients, developing service plans and conducting an individual orientation to review policies, procedures and expectation when individuals are admitted into the intensive in-home program.

    Qualifications:

    Licensed Mental Health Professional (LMHP) or LMHP Supervisee or Resident

    Certified Pre-screener

    Qualified Mental Health Professional-Adult (QMHP-A)

    Qualified Mental Health Professional-Child (QMHP-C)

    Qualified Mental Health Professional-Eligible (QMHP-E)

    Responsibilities:

    a) Assists in providing services in alignment with the treatment plan and updates

    b) Models and teaches problem-solving techniques

    c) Provides a variety of interventions in client’s’ home and community as appropriate and necessary. These interventions may be ADLs, budgeting, health and safety skills, functional skills, effective communication skills, behavioral, social and recreational, as needed

    d) Provides linkage to community-based services and supports; provides advocacy and support family with these services

    e) Role-play with client on scheduling of appointments, as needed

    f) Role-play with client on arranging transportation, as needed

    g) Available 24-hour crisis intervention support

    h) Is responsible for proper and timely documentation of all information required by the by Life Enhancement Services, LLC.

    i) All daily notes must be turn-in within 24 hours from the date of service rendered.

    j) Maintains regular and ongoing contact with referral agencies, as well as other agencies involved in client’s case

    k) Actively coordinates and plans discharge and aftercare plans with referring agency

    l) Attends interagency meetings, as necessary

    m) Attends and conduct team meetings

    n) Attends and conduct orientation, in-service training, and conferences as scheduled

    o) Attends supervision

    p) Accompany client to appointments, and in the community,

    q) Develop and implementation of Individual Service Plan (ISP)

    r) Complete screening and intake package

    s) Gather co-existing information regarding the client’s medical & psychiatric history prior to referral for clinical assessment.

    t) Make referral to other professionals if needed

    u) Performs other duties assigned in administrative department based on the availability of work designated by the CEO.

    v) Knowledgeable of Intensive In-home program

    w) Attend Human Rights Meetings quarterly and as needed.

    x) Conduct marketing for clients until agency increase growth prior to hiring a marketing director

    y) Working with clients in the home and community

    z) Coordinating transportation for your client to meet all medical and scheduled appointments

    Required Knowledge, skills and abilities

    Ability to complete First Aid, CPR training and behavior management training (TOVA)

    Valid driver’s license with acceptable driving record

    Reliable transportation with liability coverage

    Demonstrate ability to work with minimum supervision

    Good documentation and communication skills

    · Must clear criminal and sex offender background checks prior to start date

    · Must provide weekly timesheet. Keep in mind Life Enhancement Services does not authorize it employees to work over time.
