    Job Summary

    This role starts with a part-time and/or a per diem commitment* of approx. 10 patients at their residence to set the path for licensing and accreditation for the agency. During the part-time employment period, patient visit hours are flexible between 9 am to 5 pm. We are looking for a compassionate, professional, reliable Home Health Clinical RN Manager for Sterling Home HealthCare agency in Indianapolis, Indiana.

    The RN Clinical Manage must be kind, compassionate, dependable, and love caring for our Hoosier population

    Will provide overall management and supervision of client care activities within the home care setting.

    You will work in conjunction with other team members to ensure appropriate level of staffing and coordination of care in order to meet client goals.

    This position requires an ability to be flexible and work with all members of the health care team and provide ongoing supervision, clinical education, support, and evaluation to the nursing staff.

    This role may include communication of updated policies and procedures, caseload assignment, and additional clinical needs, but not limited to the following task -

    The RN Clinical Manager performs visits, assessments and supervises home health services to patients in their place of residence in accordance with attending physician orders and plans of care and strives to provide the highest quality of care

    Perform on site supervisory visits to assess client, family, environment, and clinical caregivers, complete follow-up documentation and provide the Administrator with census, staffing, team productivity, and other reports as requested.

    Provide nursing updates and obtain re-authorization for continued care

    Evaluate and ensure that competency requirements are met for all the staff

    Participate in employment decisions affecting nursing staff, including hiring and termination as appropriate.

    Participate in call for after hour’s client care

    Base decisions and actions on ethical principles and foster a non-judgmental, non-discriminatory climate in which care is delivered in a manner sensitive to sociocultural diversity

    Responsibilities

    Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans

    Administer medications and monitor patients for side effects or reactions

    Document any changes in patient conditions and report on changes

    Maintain accurate records of patient care and discuss observations with the supervisor

    Qualifications

    Diploma, Associate, or Bachelor degree in nursing from state accredited RN program

    RN licensure in the State of Indiana

    Medical Specialty: RN with 1 year home health experience

    Previous experience as a RN in home health preferred

    First aid/CPR certification required/BLS Certification while employed in a clinical role

    Valid Driver’s license and acceptable motor vehicle record

    Must be able to travel; has own transportation

    Occasional lifting, carrying, pushing and pulling, lifting of 25-50 pounds

    Excellent interpersonal and client care skills
