    We are looking for a compassionate, professional, reliable Home Health Clinical Managers for Sterling Home HealthCare agency in Indianapolis, Indiana. Must be kind, compassionate, dependable, and love caring for our Hoosier population.

    The Nurse Manager provides overall management and supervision of client care activities within the home care setting. You will work in conjunction with other team members to ensure appropriate level of staffing and coordination of care in order to meet client goals. This position requires an ability to be flexible and work with all members of the health care team and provide ongoing supervision, clinical education, support, and evaluation to the nursing staff. This role may include communication of updated policies and procedures, caseload assignment, and additional clinical needs, but not limited to:

    Responsibilities of the Clinical Manager:

    The Clinical Manager supervises home health services to homebound patients in their place of residence in accordance with attending physician orders and plans of care and strives to provide the highest quality of care

    All skilled nursing and other therapeutic services are under the supervision and direction of the Clinical Manager

    This role starts with a part-time and/or a per diem commitment* to treat a handful of patients at their residence to set the path for licensing and accreditation for the agency. During the part-time employment period, patient visit hours are flexible between 9 am to 5 pm.

    Assume responsibility in coordinating care to assigned clients, establishing a goal directed care plan from admission to discharge which includes a comprehensive ongoing assessment of clients’ needs

    Perform on site supervisory visits to assess client, family, environment, and clinical caregivers and complete follow-up documentation

    Ensure availability and proper operation of necessary equipment and supplies related to patient care

    Provide direct client care as needed

    Promote and manage expectations and satisfaction with internal and external customers

    Evaluate the quality and effectiveness of nurse practice and nursing services, analyzing appropriate data and information to identify opportunities for collaboration with all stakeholders in order to improve services and patient outcomes

    Promote efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement

    Support the coordination of interdisciplinary care

    Provide the Administrator with census, staffing, team productivity, and other reports as requested

    Identify staffing patterns and needs of the assigned team

    Provide direct clinical supervision, performance evaluations, and disciplinary action of all assigned staff

    Evaluate and ensure that competency requirements are met for all the staff

    Ensure and perform in-home supervisory visits with assigned team members

    Provide nursing updates and obtain re-authorization for continued care

    Provide ongoing supervision, orientation, training, education, and evaluation of clinical field staff

    Identify professional practice standards within the organization and identify areas of strengths as well as areas for professional practice development

    Contribute to nursing education and professional development of staff, students, and colleagues

    Participate in employment decisions affecting nursing staff, including hiring and termination as appropriate

    Maintain compliance in accordance with company policies and procedures, laws and regulations, and professional standards within the state of practice

    Maintain a professional demeanor consistent with registered nurse standards of practice

    Provide best practice in delivery of nursing care to the appropriate population and adhere to the standards of professional nursing practice

    Base decisions and actions on ethical principles and foster a non-judgmental, non-discriminatory climate in which care is delivered in a manner sensitive to sociocultural diversity

    Participate in call for after hour’s client care

    Promote an environment of quality and safe client care through participation, development, and adherence to the QA plan and associated activities and metrics

    Ability to assess clients and provide direct client care as needed

    Requirements:

    Diploma, Associate, or Bachelor degree in nursing from state accredited RN program

    RN licensure in the State of Indiana

    CPR certification while employed in a clinical role

    BLS Certification while employed in a clinical role

    Valid Driver’s license and acceptable motor vehicle record.

    Supervising Experience: 2 years (Preferred)

    Leadership Experience: 2 years (Preferred)

    RN (Required)

    Physical Requirements:

    Must be able to speak, write, read and understand English

    Must be able to travel; company does not provide vehicles or transportation

    Occasional lifting, carrying, pushing and pulling of 25 pounds

    Must be able to lift 50 pounds

    Environment:

    Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions

    Possible exposure to blood, bodily fluids and infectious diseases
