What is the specific title of the position? Inpatient Case Management Care Coordination Registered Nurse
Work location (specify UHG/UHC/Optum location or remote/telecommute)? UHC- field base telecommute Las Vegas, Nevada.
Work hours (ex. 9am-5pm day/night shifts rotating shifts etc)? Day 8:30 am-5pm
Assess and create a discharge plan to ensure patient receives quality medical care. Implement care strategies that are individualized for patients and directed toward the most appropriate and the least restrictive level of care.
2\. Utilize both company and community-based resources to establish a safe and effective case management plan for hospitalized members and effective discharge planning to prevent readmissions.
3\. Collaborate with patient, family, and health care providers to develop an individualized plan of care that encompasses both acute care episode and post hospital discharge plan.
4\. Communicate with all stakeholders the required health related information to ensure quality coordinated care and services are provided timely to all hospitalized members.
5\. Advocate for patients and families as needed to ensure the patient’s needs and choices are represented and supported by the health care team.
6\. Work performed and completed onsite at various acute and alternative level of care facilities within the area.
7\. Floating may be required covering cases in another facility telephonically.
8\. Depending on EMR access/availability may be required to cross covering in multiple facilities based on the daily average census and PTO coverage
9\. Attend all Medical Management clinical rounds.
What skills/attributes are required (please be detailed as to number of years of experience for each skill)?
Minimum two years of varied clinical experience in a hospital setting with an emphasis on case management or utilization management. Experience in a managed care organization · preferred.? · Understand spectrum of healthcare delivery system including post hospital care such as SNF/LTAC/AIR criteria · Must possess a valid Nevada driver-s license and maintain personal auto insurance coverage. Must reside in the state of Nevada.
