Large managed care company is looking for a RN Case Manager for patient case management, clinical assessment, face to face patient care/triage, transition of care following ER and Hospital admissions, patient touchpoints by phone and administrative tasks

WORK LOCATION: 10616 Main St, Thonotosassa, FL 33592

PAY: $40.00

CONTRACT DATES: 12/23/24 - 4/21/25

HOURS: M-F, 8-5 (40 hrs/wk)

RESPONSIBILITIES

1\. Works with the providers and clinic staff to identify patients at high risk.

2\. Supports longitudinal care of the patient with chronic care conditions by:
a. performing assessment of health conditions
b. performing medication reconciliation
c. conducting Motivational Interviewing and Self-Management Goal setting
d. providing patient education
e. creating referrals to appropriate agencies and resources

3\. Supports transition of the patient with chronic care conditions from inpatient to outpatient setting, by:
a. performing assessment of transitional needs
b. performing medication reconciliation
c. establishing and reviewing contingency plan
d. providing patient education
e. assisting with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments

QUALIFICATIONS

1\. Bachelor’s degree in Nursing or Associate’s degree in Nursing with four or more years of experience as an RN

2\. Registered Nurse with an active and unrestricted license to practice in FL

3\. Current BLS certification

4\. Two or more years of experience in a physician’s office, clinical or hospital setting

5\. Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD

6\. TB blood test, MMR, Varicella, Hep B titers, respirator medical clearance required

6\. Fluent written and verbal skills in English and Spanish (preferred)
