Plan, organize and direct home care services and care plan development
Ensure high quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on client needs
Provide coaching and education to patients and their caregivers
Build relationships with case managers and other referral partners to ensure the quality of client care.
