32hrs a week | Tues-Fri | Long Weekends Off

Responsibilities of RN Case Manager:
Provides care as defined within the scope of the state’s nurse practice act.
Makes the initial evaluation visit and regularly reevaluates the client’s nursing needs.
Prepares clinical and progress notes and summaries of care.
Serve as backup for Critical Care Clients, In-School Staffing & PDN
Assures proper maintenance of clinical records in compliance with local, state and federal laws.
Helps educates and coach care-giving staff and assists with new hire clinical training, as needed.
Participate in monthly clinical on-call rotation.

RN Case Manager Requirements:
Current unrestricted RN license in Minnesota.
Certified CPR
Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days)
Minimum two (2) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred.
Critical Care Experience (1-2 years)
Reliable car/auto insurance.
Clean background and criminal record.
Understands regulatory compliance with state and federal regulations.
Ability to work on-call rotation with compensation.
Expected hours: 32 per week
