    Qualifications:

    Required:

    Associate degree or higher

    Valid Registered Nurse (RN) license in Ohio (OH)

    1+ years of experience in Medical-Surgical (Med-Surg) or Home Health Care

    1+ years of experience in Burn Unit/Wound Clinic or Wound Care

    Valid American Heart Association Basic Life Support (AHA BLS)

    Overview: We are seeking a full-time RN Case Manager for Home Health in East Cuyahoga County, OH. The ideal candidate will provide skilled nursing care, demonstrate effective communication and collaboration, and maintain patient safety. This role requires strong organizational skills, the ability to work independently, and a commitment to maintaining confidentiality. The candidate should have experience in home health care and will service areas including Bratenal, East Cleveland, Cleveland Heights, University Circle, and surrounding areas.

    Key Responsibilities:

    Provide direct or indirect skilled comprehensive nursing care

    Utilize nursing process for data collection, planning, and evaluation

    Implement prevention, therapeutic, and rehabilitative nursing care

    Communicate and collaborate effectively with healthcare providers, third parties, patients, and families

    Maintain awareness of patient comfort and safety needs

    Observe patients, record conditions and reactions, and notify supervisor or Physician of significant incidents

    Respond to life-saving situations based on nursing standards and protocols

    Use oral and written communications to convey pertinent information to the healthcare team

    Schedule and Shift Details:

    8:30 AM to 4:30 PM, with 1 weekend per month

    Includes working one summer and one winter holiday
