  • Responsibilities

    The Case Manager is typically a professional ensuring that patient care is coordinated and managed appropriately;

    Case Managment is incharge of staffing and plotting visits for SN (skilled nurses), MSW, CHHA, call patients after SOC/RECERT/ROC, Assign Supervisory visits for LVN’s and CHHA’s, ensure timely submission of the Oasis/visit notes, coordinate lab/pharmacy/meds/staff/MD/track/obtain authorization for HMO, coordinate DME orders and supplies, ensure a CHHA plan is present if applicable, inform visiting staff when patient is transferred to Hosp/SNF and after ROC.

    REQUIREMENTS:

    One (1) year expeirence in home health and or case amangement.

    Must be able to pass a background check

    Eligible to work in US

    Basic Microsoft Office Excel, Word, Anti Virus

    Strong attention to detail - note taking

    Strong organizational skills

    Positive Attitude - Team Success

    Willingness to study, train and learn skills

    On-Call company cell phone, after hours (rotational)

    The Candidate selected for this position must speak and write clearly in English being bilingual is always a plus, be comfortable to communicate quickly and efficiently via email, and text.
