Job Description
The Case Manager (CM) is responsible for the development and implementation of effective and on-going Case Management Utilization Review, Discharge Planning Process, Workers Compensation, and assisting with Quality Improvement Initiative Projects and Quality Improvement/Clinical Education programs; Responsible for the efficient and competent case management of inpatients, swingbed, and observation patients. Develops policies and procedures for areas of responsibility.
The CM is also responsible for filling in vacant positions that fall within the nurse practice act for registered nurses/ licensed practical nurses.
The CM is responsible for excellent communication and management of resources and upholding excellent clinical standards of care.
Qualifications
1. A Florida licensed Registered Nurse or Practical Nurse
2. Minimum 2 years acute care nursing or skilled nursing facility experience
3. One year Utilization Review/Case Management/ insurance verification /authorization Experience required
4. Certification in Case Management preferred
5. One year EHR experience
6. Current CPR and ACLS (ACLS within six months of hire date
7. IV certification within 6 months of hire
8. Excellent written, computer, and verbal communication skills with a strong ability to comprehend and follow provider orders for safe patient.
9. Excellent interpersonal skills with the ability to interact effectively and professionally with people at all socio-economic and organizational levels.
10. A hands-on approach to duties.

12. Excellent internal and external customer service skills
