    Qualifications Required:

    Associate degree or higher.

    Minimum of 1 year of experience in both Medical-Surgical (Med-Surg) and Home Health Care.

    Valid Registered Nurse (RN) license in Ohio (OH).

    Valid American Heart Association Basic Life Support (BLS) certification.

    Preferred: Minimum of 1 year of experience in Wound Care or Burn Unit/Wound Clinic.

    Preferred:

    Bilingual proficiency in Spanish.

    RN License Number.

    Any specialized certifications.

    Overview: We are currently seeking a full-time RN Case Manager for Home Health in West Cuyahoga County, OH. The ideal candidate will deliver skilled nursing care, exhibit effective communication and collaboration skills, and prioritize patient safety. This role requires strong organizational abilities, the capacity to work autonomously, and a commitment to maintaining patient confidentiality. Experience in home health care is preferred. The coverage areas for this position include Parma, Middleburg, Brooklyn, Brookpark, Fairview Park, Westpark, and surrounding areas.

    Key Responsibilities:

    Provide direct or indirect skilled comprehensive nursing care to specified age and diagnostic groups.

    Utilize the nursing process for data collection, planning, and evaluation of nursing care.

    Demonstrate nursing knowledge and skills in implementing prevention, therapeutic, and rehabilitative nursing care.

    Utilize effective interviewing techniques, collect data, and maintain awareness of patient comfort and safety.

    Communicate and collaborate effectively with healthcare providers, patients, families, and third parties.

    Observe patients, record significant conditions, reactions, and notify supervisors or physicians of changes.

    Respond to life-saving situations according to nursing standards, policies, procedures, and protocols.

    Use oral and written communication to convey pertinent information to the healthcare team.

    Schedule and Shift Details:

    Shift: 8:30 am to 4:30 pm, with one weekend per month.

    Requirement to work one summer and one winter holiday.
