Large management company looking o fill a CONTRACT RN role in Baltimore, MD.

Contract end date- Dec 06 2024

Duties:

Duties Demonstrates expert clinical skills and provides complex nursing care to patients usually in a specialized care setting. Serves as a member in decision-making committees. RN will be conducting face to face and telephonic visits on complex Medicare advantage members in and around the Baltimore area. RN will assist an established team of NPS see a group of pts in our risk program. RN will also be visiting members who have been recently discharged home from a hospital admission. The goal is to stabilize pt. in the home post D/C and prevent readmission where possible. Referrals will be made to case management, pharmacy and social work as needed. RN will work with established NP team to determine next care plan steps for pt.

Skills:

Good driving record, no issues with driving in and around Baltimore as part of this job Good communication skills with Health care team including escalating issues to pt. PCP Great customer service skills comfortable taking care of pts in the home setting Great triage skills Case management/ insurance experience helpful able to navigate EMR, laptop, cell phone, WebEx, outlook, VPN connection, some excel comfort Comfortable with assessment of social determinants of health. Linking pt. with community resources Skilled in pt. teaching and education. Red flag/ exacerbation teaching especially around chronic conditions Comfortable with elderly/ frail pts. Some pts homebound
