Job Description: Our Home Health Registered Nurses provide in-home assessments and treatments, creating personalized Care Plans that drive positive patient outcomes. In this role, you’ll visit patients at their residence, develop and manage individualized Plans of Care, and utilize a full range of clinical skills, including wound care for various types, ostomy management, PICC line care, IV infusions, and more. This position is ideal for an RN with a comprehensive clinical background who is passionate about delivering high-quality, patient-centered care in a home setting.
Conducting thorough patient assessments (Oasis E) to develop personalized care plans
Administering skilled nursing care in accordance with physician's orders and established protocols
Monitoring and documenting patient progress, ensuring accurate and timely charting
Educating patients and their families on disease management, treatment plans, and self-care techniques
Collaborating with physicians, therapists, and other healthcare professionals to coordinate and ensure seamless care transitions
Participating in case conferences and interdisciplinary team meetings to review patient progress and adjust care plans as needed
Complying with all regulatory guidelines, including OASIS assessment and documentation
Qualifications:
Active Registered Nurse (RN) license in the state of Florida
Minimum of 2 years of experience in home health care (required)
Proficiency in OASIS documentation, required
Ability to exercise initiative and independent judgment
Strong clinical and assessment skills
Excellent communication and interpersonal skills
Proficient computer skills for accurate documentation and electronic medical records
Reliable transportation and valid driver's license
