We are seeking an RN/LPN to join our team! You will be responsible for the assessment, diagnosis, and treatment as ordered of assigned patients.
Responsibilities:
To provide nursing care in accordance with the Clients plan of care to include comprehensive health and psychosocial evaluation, monitoring Client
s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care
Coordinates total client care by conducting comprehensive health and psychosocial evaluation, monitoring client`s condition, health promoting sound preventive practices, coordinating services and teaching and training activities.
Monitor and report changes in patient symptoms or behavior
Communicate with collaborating physicians or specialists regarding patient care
Educate patients and family about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records, submit clinical and progress notes timely, and presents to clinical record review
Provide advice and emotional support to patients and their family members
Participates in Client`s case conference and discharge planning process
Qualifications:
Must be a graduate from an accredited school of nursing and licensed as a registered or practical nurse in North Carolina
Minimum of one year experience
Previous experience in nursing or other medical fields
Familiarity with medical software and equipment
Ability to build rapport with patients
Strong problem solving and critical thinking skills
Ability to thrive in a fast-paced environment
