    Green Meadows Home Health is looking a Full Time RN to join our QA team.

    QAPI Coordinator supports the department’s efforts to track and trend data outcomes, performance improvement initiatives, infection control program and education/staff development. Responsible for data collection of quarterly performance improvement reports and administrative/secretarial duties relation to the quality department.

    Essential Job Functions/Responsibilities Responsible for the implementation and monitoring of the organization’s quality improvement program. Ensures that there are processes in place to monitor and measure all activities related to performance improvement. Coordinates activities related to clinical record review for data collection. Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction. Reports information to Director of Quality.

    Works collaboratively with Clinical Team Managers to assure documentation is complete and consistent with care and reflects legal requirements. Establishes a process model for improvement and provides education and training to organization personnel on the performance improvement plan, as well as team development. Under the direction of the Director of Quality, implements the Agency’s continuous Quality Assessment and Performance Improvement (QAPI) plan and program. Collates, tracks and compiles quality indicator data including, but not limited to:Incidents/occurrencesInfections Home Health JAYCO/Patient satisfaction submission. Program process & outcome measures. All Performance Improvement focus group data Manages quarterly clinical record review process and ensures timely and accurate results. Tabulates and submits findings to the Director of Quality.

    Job Title/Position: QAPI Coordinator Supports all quality and performance improvement functions. Coordinates committee meetings, maintains minute logs and distributes meeting minutes. Performance Improvement Participates in accreditation activities, and state and federal survey preparedness and follow up. Submits reports as needed to the Director, with clerical/scheduling responsibilities necessary for Education/Staff Development program. Works with Clinical Education on areas for staff ­­­­­­­­­development and areas for education improvement. Performs other activities as assigned by the Director of Quality.     Position Requirements | Position Qualifications One (1) to two (2) years of clinical home health experience required. RN License Working knowledge of OASIS and ICD-10 coding. OASIS certification preferred; OASIS Home Health ICD-10 coding certification preferred. Knowledge of federal regulations and state licensure requirements. Working knowledge of computer systems. Excellent coordination and communication skills. Detail oriented and able to work with minimal supervision.
