Job Description:

Glades Talent is working with a leading national home healthcare agency in Garden City, seeking a Full-Time Field Registered Nurse Case Manager to provide:

-Responsible for overseeing client services.
-Develops a plan of care for assigned cases, starts cases, and is the point person to families.
-Communicates regularly with other team members to ensure client satisfaction and safe practices.

-Supervisory check-in visits throughout the Garden City service area for the geriatric patient populations in their homes.

JOB REQUIREMENTS:

* Must have a minimum of 3 years' of experience as an RN

Must have management/supervisory experience

Must have geriatric experience

* Must be fully COVID vaccinated

* Must own a vehicle and OK commuting to patients' homes
