Work closely with healthcare providers, case management agencies, and clients to facilitate the referral process for the Medicaid Waiver Programs, Insurance companies and VA, schedule appointments, and provide necessary information to the Case Management Agencies, Insurance companies and VA. This job requires excellent communication and organizational skills, as well as knowledge of medical terminology and procedures.
Job Duties
o Collaborate with healthcare providers, case management Agencies and Area on Aging
o to ensure timely referrals for clients.
o Maintain detailed records of client referrals and follow-up appointments.
o Communicate with clients to update them on the status of their referrals and scheduled assessments with the Nurse.
o Proactively reach out to clients who miss scheduled assessments and assist in rescheduling.
o Ensure that clients receive the necessary paperwork to start the waiver process, monitor paperwork being returned. Communicating with physician offices to ensure medical records are being returned timely.
o Facilitate the exchange of medical information between different healthcare providers as needed.
o Assist in the development and maintenance of referral guidelines and policies.
o Meeting monthly with the marketing team
o Marketing and/or building relationships with the Case Management Agencies.
o Setting up at marketing events as needed.
