Requires prolonged walking and standing, frequent bending, stooping or stretching. Ability to lift up to 30 pounds occasionally. Requires corrected vision and hearing to normal range. Manual dexterity and eye-hand coordination required for use of computers, phones, computers and other equipment.
Day to Day:
• Facilitating authorization for care, and transfer of necessary information to assure YNHS patients get care in a timely manner.
• Make appointments, facilitate interpreters, transportation as necessary, to reduce barriers for patients.
• Obtain authorizations as needed from third parties.
• Provide complete documentation in patient records.
• Assist in patient tracking efforts to assure care is completed and providers are informed as needed.
• Communicate information to patients, specialists and outside providers as directed.
• Manages and documents referrals from YNHS providers, including working from the EHR “queue” on a daily basis.
• Prioritizes workflow and maintains appropriate work volume according to established procedures and/or as directed by supervisor.
• Protect Health Care Information, consistent with medical, ethical, and legal requirements.
• Work with Health Care Information staff to assure appropriate reviews are completed before releasing or filing patient information.
• Maintain organized, clean, efficient and confidential work area.
• Perform related work and other tasks as requested.
